Developing your care plan

  • Ongoing review and adjustment: Care is actively managed through multidisciplinary team reviews. Physicians, nurses, therapists, and case managers evaluate progress and adjust treatment goals as needed. Individuals can expect continuous reassessment versus a static treatment plan.

Who you will meet with

  • Counselors and therapists: Clients work with a primary counselor for individual support and treatment planning, while also participating in group therapy sessions. Sessions focus on recovery skills and treatment progress, with access to a broader counseling team across the program.
  • Medical and clinical team: Physicians and nurses support medical and mental health needs throughout treatment that includes monitoring symptoms and providing appropriate care.
  • Case managers: Case managers assist in coordinating care beyond treatment. This can include discharge planning, connecting clients to social services, and supporting next steps beyond the program.

Typical schedule

  • Program schedule and structure: The partial hospitalization program (PHP) runs Monday through Friday from 9:00 a.m. to 4:30 p.m., providing full-day structured care. The intensive outpatient program (IOP) meets 3-5 days a week from 9:00 a.m. to 12:00 p.m., with additional evening telehealth IOP meetings available Monday through Friday from 5:00 p.m. to 8:00 p.m. for individuals balancing work or daytime responsibilities.
  • Therapies and support: Daily programming includes group therapy using approaches such as Cognitive Behavioral Therapy and Dialectical Behavior Therapy, in addition to psychoeducation and recovery skills that focus on relapse prevention and substance use education. Clients may also participate in specialized or experiential therapies, including Bio-Sound therapy and Recovery-Oriented Challenge Therapy (ROCT), in addition to track-specific groups. Individual therapy is scheduled alongside group sessions and weekly family therapy. Education sessions are offered on Thursday evenings.

Aftercare

  • Discharge Planning and Continuity: Aftercare planning begins before discharge, with structured planning services that help clients prepare for the next phase of recovery. Individuals may transition into continued care with River Oaks or the American Addiction Centers network, allowing for step-down support without changing providers or treatment approach.
  • Ongoing support and resources: Aftercare plans may include continued outpatient therapy and lower levels of care, referrals to counseling and mental health services, and connection to peer support groups. Case management supports meeting practical needs such as housing and social services. Discharge education may include naloxone and overdose education to reduce risk post-treatment.
